Don't have a prescription?
Under HIPPA regulations, patients have the right to request a prescription from their physician. If you have difficulty accessing your prescription, please contact our Customer Service team.
Prescriptions can be handwritten or typed and should include;
- Physicians name
- Contact information
- Signature of Care Provider
- Your name
- Statement of Care required
